I give permission for my child(ren) to participate in Castle Valley Civic Ballet’s production of The
Nutcracker (the Production). My child (the Student) agrees to attend all rehearsals conducted by Castle
Valley Civic Ballet associated with The Nutcracker. I have read and agree to the contents of the
Participation Agreement Form requirements.
Further, I understand and accept that Castel Valley Civic Ballet assumes no responsibility for
injuries or illness that may occur as a result of the Student’s participation in the Production. I expressly
acknowledge, on behalf of myself and my heirs that I assume the risk for any or all physical injuries,
disability, loss, damage, or illness that may result from the Student’s participation in the Production.
In consideration of the opportunity to participate in the Production, I hereby release and will hold
harmless Castle Valley Civic Ballet and all of its officers, employees, directors, attorneys, contractors,
volunteers, dancers, and agents from any and all liability, actions, causes of action, debts, claims and
demands of every kind and nature, whether known or unknown, contingent or non-contingent, and
specifically including any claim for negligence or negligent acts, but in no event for gross negligence or for
willful or wanton misconduct, which I now have or which may arise out of, or in connection with, the
Student’s participation in the Production or the above Participation Agreement.
I represent that the Student is in good health and physically capable of participating in all
rehearsals and performances. I accept responsibility for obtaining appropriate accident, health, and
hospitalization insurance to cover the student in the event of personal injury. In the event of an injury or
other medical emergency in which I cannot be reached, I authorize Castle Valley Civic Ballet to seek any
medical assistance reasonably required. I agree to be responsible for medical expenses incurred on
behalf of the Student during the Production.

Castle Valley Civic Ballet has put in place preventative measures to reduce the spread of COVID-
19; however, Castle Valley Civic Ballet cannot guarantee that you or your child(ren) will not become

infected with COVID-19. Further, attending Castle Valley Civic Ballet could increase your risk and your
child(ren)’s risk of contracting COVID-19. By signing this agreement, I acknowledge the contagious
nature of COVID-19 and voluntarily assume the risk that my child(ren) and I may be exposed to or
infected by COVID-19 by attending Castle Valley Civic Ballet rehearsals/performances, and that such
exposure or infection may result in personal injury, illness, permanent disability, and death. I understand
that the risk of becoming exposed to or infected by COVID-19 from Castle Valley Civic Ballet may result
from the actions, omissions, or negligence of myself and others, including, but not limited to, Studio
employees, volunteers, and program participants and their families. I voluntarily agree to assume all the
foregoing risks and accept sole responsibility for any injury to my child(ren) or myself (including, but not
limited to, personal injury, disability, and death), illness, damage, loss, claim, liability, or expense, of any
kind, that I or my child(ren) may experience or incur in connection with my child(ren)’s attendance at
Castle Valley Civic Ballet or participation in Studio programming. On my behalf, and on behalf of my
children, I hereby release, covenant not to sue, discharge, and hold harmless Castle Valley Civic Ballet, its
employees, volunteers, agents, and representatives, of and from the Studio, including all liabilities, claims,
actions, damages, costs or expenses of any kind arising out of or relating thereto.
I further understand and agree that Castle Valley Civic Ballet is not responsible for personal
property that may be lost or stolen during the Student’s participation in the Production.
I also give my consent to Castle Valley Civic Ballet to produce and use, without charge, limitation
or condition, photographs, film, footage, or other audio or video recordings that may include the
Student’s image, personal or voice for purposes of promoting, marketing or interpreting Castle Valley
Civic Ballet programs, services and/or productions worldwide. Prior to signing this document, I have had
an adequate opportunity to read and understand it, have had an opportunity to ask questions about it,
and my questions have been answered to my satisfaction.
